Memory Baseline is a screening and education starting point. It is not a medical clinic.
What we collect
If you download the Memory Baseline Guide or schedule an MCI Screen conversation, we may collect your name, email address, and optional phone number. We may also note whether you are exploring this for yourself or as a caregiver.
What we do not collect
Do not send symptoms, diagnoses, medications, test results, or anyone else’s health details through our forms. Ordinary forms on this site are not for medical records.
How we use contact details
We use them to send the guide and, if you want, follow-up education about Memory Baseline and how to schedule the MCI Screen. We do not sell this information.
