What to bring to your visit

You do not need perfect words. You need a short list and, if you have one, a dated Memory Baseline report.

  1. Write one example of a memory change you have noticed. Keep it factual. Do not diagnose yourself.
  2. Write when it started, if you know.
  3. List the medications and health history you already share with this clinician. Do not send that list to Memory Baseline.
  4. Bring your MCI Screen report if you have one.
  5. Ask: “How should we use this baseline at this visit?”
  6. Ask: “What should we watch, and when should we talk again?”

Bring this to a qualified clinician. Memory Baseline does not diagnose or replace a medical evaluation.

If you have sudden confusion, weakness, severe headache, speech changes, or other urgent symptoms, seek emergency medical care.
